Understanding Vision Loss: AMD, Glaucoma, and Retinitis Pigmentosa

Maintaining vision health and navigating a diagnosis of a progressive eye condition requires a clear understanding of how different diseases affect the ocular system. While many conditions lead to vision impairment, the physiological impact on the visual field varies significantly. This guide provides an overview of Age-related Macular Degeneration (AMD), Glaucoma, and Retinitis Pigmentosa (RP), focusing on their causes, symptoms, and typical patterns of progression.

Someone with macular degeneration has lost the center of their visual field but often retains the edges. Someone with glaucoma has the opposite problem. Those two people need different things, and here is what each of the major conditions does to sight.

1. Macular Degeneration: Losing the Centre

Age-related macular degeneration (AMD) is a condition that primarily affects the macula, a small but vital area in the center of the retina. The macula is responsible for "straight-ahead" vision and the sharp detail required for tasks like reading and facial recognition. AMD occurs in two forms: dry and wet. Dry AMD, the most common form, involves the gradual thinning of macular tissues and the accumulation of drusen (small yellow deposits). Wet AMD is more severe and occurs when abnormal blood vessels grow underneath the retina, leaking fluid or blood and causing rapid damage to central vision.

The specific nature of vision loss in AMD is the development of a central scotoma—a blind spot or area of significant distortion in the middle of the visual field. While peripheral vision typically remains intact, allowing individuals to maintain some spatial awareness, the loss of high-resolution central vision makes detailed work increasingly difficult as the condition progresses.

Stargardt disease is a juvenile form of macular degeneration with the same pattern of central loss, usually appearing in childhood or adolescence.

2. Glaucoma: Losing the Edges

Glaucoma refers to a group of eye conditions that damage the optic nerve, the health of which is vital for good vision. This damage is often caused by an abnormally high pressure in the eye (intraocular pressure). Glaucoma is a leading cause of blindness for people over 60, but it can occur at any age. The most common form, open-angle glaucoma, often has no symptoms other than gradual vision loss.

Unlike AMD, glaucoma typically erodes vision from the outside in. This loss of peripheral (side) vision creates a "tunnel vision" effect. Because the loss is gradual, many people do not notice a change in their sight until the condition is at an advanced stage. In the final stages, the central field may eventually be affected if the optic nerve damage continues to progress.

3. Retinitis Pigmentosa (RP): Genetic Peripheral Decay

Retinitis pigmentosa (RP) is a group of rare, genetic disorders that involve a breakdown and loss of cells in the retina—the light-sensitive tissue lining the back of the eye. RP changes how the retina responds to light, making it difficult to see. Symptoms usually begin in childhood or adolescence. The first sign is often "night blindness," where the individual finds it difficult to adjust to low-light environments.

As RP progresses, the light-sensing "rod" cells (responsible for peripheral and night vision) deteriorate first, followed by the "cone" cells (responsible for central vision and color). This leads to a progressive narrowing of the visual field, eventually resulting in severe tunnel vision. While the rate of progression varies by individual and genetic subtype, the long-term impact is a significant reduction in the usable field of view, making independent navigation a primary challenge.
